Funding requests  for medical equipment

Please complete the attached funding application form, including quotations from three different suppliers and a supporting letter from the head of your institution. If three supplier quotations are not available, kindly explain the reason in the "Other Information" section of the form.

The Charity presently aims to fund items up to a maximum total of £4,000 per annum. However, the Committee may consider applications for items exceeding this amount if they are deemed suitable for future fundraising initiatives and are consistent with the Charity’s stated aims and objectives. Please note that, in accordance with its standard policy, the Charity does not make cash donations under any circumstances.

Applications that are incomplete will not be considered. In specific instances, a member of the Charity’s local committee in Sri Lanka may contact the applicant to request additional information. Wherever feasible, items will be procured within Sri Lanka to ensure the availability of local support for repairs and maintenance. Applicants are expected to conduct a thorough assessment of the equipment’s compatibility and its appropriateness for use within the hospital setting. All correspondence and inquiries should be directed to the Secretary of the Charity via the medicalaidtosrilanka@googlegroups.com email address.

Please also note: Should the MASL Committee approve the donation of equipment, a certified copy of the official hospital inventory record endorsed by the head of the hospital must be submitted to the Secretary of Medical Aid to Sri Lanka within eight weeks of receipt of the equipment via the above email address.
